Application Information

If you are interested in having your child participate in Head Start, please fill out the pre-qualifying form. Staff will review the form and give you a call. If you qualify, you will be given an application appointment. If you are over-income and do not qualify, you will be asked for further information. If you have questions, be sure to call your local Head Start center or call 801-972-2337 or for Tooele/Wendover 435-882-6743. Calls are answered within 5 working days.

Head Start enrollment is based on the income levels of participating families. The US Government establishes these guidelines annually, known as the "Federal Poverty Guidelines" and they can be found below. Ninety percent of Salt Lake CAP Head Start's enrollment must meet these income qualifications; the other ten percent are reserved for children and families with special needs. Please call the center nearest you for more information or fill out the prequalifying application here online and you will get a call back.
